If you are applying for your first ever Passport

If you are applying for your first OFFICIAL Passport but have a TOURIST Passport

BOX A: ITEMS REQUIRED:

· DS11 application form (see overleaf)

· 2 x passport photos (Kodak in BX or Electric Ave: no military uniform, collared shirt required)

· Photo ID: Military ID card or Driving License

· Original Birth Certificate/CRBA/Naturalization Cert

· Military: MFR from your UDM or CC stating why you need an Official passport which includes duration and country visiting

Civilians: Employment Verification Letter from Senior Civilian Lead

BOX B: ITEMS REQUIRED:

· DS82 application form (see overleaf)

· 2 x passport photos (Kodak in BX or Electric Ave: no military uniform, collared shirt required)

· Photo ID: Military ID card or Driving License

· Tourist Passport (we will make a copy)

· Military: MFR from your UDM or CC stating why you need an Official passport which includes duration, country visiting and if you are retaining your tourist passport

Civilians: Employment Verification Letter from Senior Civilian Lead

If you are applying for a renewal of your OFFICIAL and do not hold any other Passport or you hold a TOURIST which is older follow instructions in Box C.

If you are applying for a Renewal of your OFFICIAL and you have a TOURIST Passport which is more recent.

BOX C: ITEMS REQUIRED:

· DS82 application form (see overleaf)

· 2 x passport photos (Kodak in BX or Electric Ave: no military uniform, collared shirt required)

· Photo ID: Military ID card or Driving License

· Expiring/Expired Official Passport

· Military: MFR from your UDM or CC stating why you need an Official passport which includes duration, country visiting

Civilians: Employment Verification Letter from Senior Civilian Lead
