Monday - Friday 7:30 a.m. – 4:30 p.m.

AD Members can retrieve copies of their shot record at the link listed below. They can also get shot records for dependents however due to HIPAA requirements this system is programmed to allow access to shot records of dependents 16 years of age and younger, please contact your MTF immunization clinic for dependents age 17 and older.

Clinic services:
Immunizations for active duty members and their dependents

General administrative information:
The clinic conducts unit training the fourth Thursday of each month from 07:30 a.m. -1:00 p.m.

No appointment is needed for immunizations.

Patient Instructions:

PPD (TB) skin tests are not placed on Thursdays or on Fridays that fall before a 3-day weekend.

Yellow Fever vaccinations are given on Wednesday at 7:30 a.m. A group of three or more in needed for yellow fever vaccinations.

NOTE: All patients requiring vaccines for deployment or travel must visit Public Health. Travel vaccines not related to deployment require a prescription from your primary care provider after screening by Public Health. Please start travel vaccination process at least a month in advance of anticipated travel.

Forms: Please bring vaccine records with you, so that all vaccinations can be annotated in your electronic medical record.
